How would you recommend purchase of the PDF version of the book for someone that refuses to use PayPal (due to trust/reputation issues, including past experiences)?

These transactions are handled by a company called FastSpring, which has been very reliable in my experience, and I don’t believe you should have any problems with the transaction.
